Dear Colleagues, As you may know, this past Spring the Keystone Beacon Community, one of 17 Beacon Communities ONC has funded across the country to demonstrate meaningful improvements in health, care and cost, launched an exciting new tool for electronically sharing patient information from skilled-nursing facilities and home health agencies. While robust information sharing with these partners is crucial to driving better outcomes for some of the most complex, costly patients, lack of adoption of health information technology has emerged as a major challenge to integration of these partners in community wide care coordination efforts. The tool, KeyHIE Transform™, addresses this gap by extracting clinically relevant information from the Minimum Data Set (MDS 3.0) or the Outcome and Assessment Information Set (OASIS) which these entities are required to submit to CMS, and converts it into a standards-based continuity-of-care document (CCD), which can be incorporated into any health-information exchange (HIE) for access by other clinicians. In addition to making the HL7-balloted implementation guides publicly available, the Keystone team has partnered with BridgeGate International to distribute KeyHIE Transform™ via a low-cost online service to any community across the country seeking to facilitate information exchange with LTPAC providers. We are pleased to offer two one-hour webinars in September to learn more about KeyHIE Transform™, and invite you, your colleagues, and other stakeholders across your community to join us during the following times:
During the webinar, Jim Younkin, Director, Information Technology at Geisinger Health System, and Program Manager for the Keystone Beacon Community, will provide a presentation on the basics of KeyHIE Transform™, an update on how the tools is being used today, and information on how you can get involved and be among the first communities in the nation to benefit from this new service.
